Eligibility

The following individuals are eligible for the vaccine, as outlined in the Executive Order issued by Governor Ron DeSantis:

  • Long-term care facility residents and staff
  • Persons 18 years of age and older
  • Health care personnel with direct patient contact
  • K-12 school employees and child care workers
  • Sworn law enforcement officers 50 years of age and older
  • Firefighters 50 years of age and older
  • Persons determined by a physician to be extremely vulnerable to COVID-19

Frequently Asked Questions

  • No appointment is necessary at this time.
  • Please note that hours may vary based on availability.
  • At this time, TFHC is administering the Moderna vaccine, unless otherwise specified.
  • Please bring identification and proof of employment as necessary.
  • Persons determined by a physician to be extremely vulnerable to COVID-19 are asked to present the Florida Department of Health's COVID-19 Determination of Extreme Vulnerability form completed by a physician prior to receiving the vaccine.

At this time, rapid testing for COVID-19 is available for all individuals at all 14 TFHC locations. Please click here for a complete list of TFHC locations.

PCR testing for COVID-19 is available by appointment only to all established TFHC patients at the following TFHC locations:

Testing is available from 1:00 PM – 5:00 PM, Monday – Friday.


 

Tampa Family Health Centers' Response to COVID-19

All 14 Tampa Family Health Centers locations are open and operating under normal business hours Monday - Friday, including weekend hours available at select TFHC locations.
